In order to prevent any confusion about your rental, please read carefully and fill out the confirmation form below. We
do everything we can to be up front about how clean our units are when you receive them, the prices we charge for all of
our equipment, and the deposits you pay to ensure your rental will be waiting for you. We also want you to be clear as
to what you will need by way of vehicle, man power, and rental pick up and return times. If you do not understand any of
the items on the form, please feel free to contact us now. We want your party or event to go as smoothly as possible.
If someone is charging the rental or deposit on a credit cared and are not coming to the warehouse to sign
for the charge, we need a faxed or emailed authorization from the credit card holder's email address
allowing us to charge their card. The authorization should read:
I (card holder's name) authorize Affordable Moonwalk Rentals, Inc. to charge my card ending in (last 4 digits)
in the amount of (amount charged). For the rental of (date).
